We are excited to invite applications for an Intermediate Care and Community Therapy Lead to join our integrated services in Newton Abbot, a welcoming market town at the heart of South Devon. This is a key leadership role within our locality-based Health & Wellbeing team, supporting people to remain independent, avoid unnecessary hospital admissions, and receive care closer to home.
Main duties of the job
In this role, you will provide supportive and visible leadership to the Intermediate Care and Community Therapy team, working closely with colleagues across health, social care, GP practices, and commissioned care providers.
Provide inspirational clinical and professional leadership to the Intermediate Care team within the locality
Play a key role within the locality management team, shaping service delivery and development
You will work closely with GP practices and commissioned care homes to deliver safe, effective Intermediate Care across community and bed-based settings.
Lead integrated, multidisciplinary working focused on supporting people in their own homes and community settings
The role includes some operational oversight beyond the locality, working alongside other leads across South Devon.
This is a varied and rewarding role that balances clinical leadership, operational management, and service improvement, within a supportive and well-established integrated team.
