We are looking for an enthusiastic and compassionate Registered Nurse to join our community nursing team, covering Ashburton, Buckfastleigh, Dartmouth, and Totnes. This is a fantastic opportunity to provide high-quality, patient-centred care in the beautiful surroundings of South Devon, where you will work alongside a supportive multi-disciplinary team on a part-time permanent contract of 30hrs per week. Main duties of the job

  • To provide high quality evidence based nursing care in a community setting.
  • To actively participate in the planning and discharge of complex patients from secondary to primary care

  • work as part of multi-disciplinary team and in partnership with primary care and secondary care.

  • Can provide motivational and negotiation skills in areas where consent/ co-operation is required or there may be barriers to understanding. To include undertaking best interest decisions
  • To ensure timely and effective communication with the team manager and other team members on professional matters
  • To liaise with patients/relatives who could be unpredictable/or challenging in their responses because of stress related problem.
  • Strong communication is essential, as we work closely with a wider multi-disciplinary team, particularly our Intermediate Care Team, to support seamless, integrated care.

  • You must have an ability to provide and receive complex, sensitive and contentious information.

  • Empathy and reassurance skills

  • Ability to provide motivational and negotiation skills in areas where consent/ co-operation is required or there may be barriers to understanding.

  • Proven relevant experience in a range of clinical skills including complex and specialist skills
  • Relevant I.T. knowledge/experience
