Job summary
You will work as a member of the Cardiac Rehabilitation team (CR), to develop and deliver high quality Cardiac Rehab service programme for patients with a wide range of cardiac conditions in line with national standards.
You will support the cardiac rehab team in visiting cardiac wards to offer advice pre/post cardiac event/surgery, encouraging uptake of CR once discharged to improve recovery and lifestyle.
Assist the Exercise specialist in organisation of community venues, the preparation of venues and resources for exercise sessions and the support of group participants.
Assist the Specialist nurses in assessing and accurately recording clinical information such as weight, height, BMI, risk factors and BP within specialist led clinics and face-to-face exercise group programme sessions.
Be involved with the implementation of MS teams weekly CR education, setting up links, emails and liaising with Dietician, Counsellors, CR team and patients/relatives/carers.
Carry out assigned tasks and duties that support and compliment the work of the CR team including basic administrative tasks, supporting patients via Telephone post cardiac event whilst liaising with relatives and multidisciplinary team.

Main duties of the job
Support the logistical organisation and running in Hospital, Outpatients, and community-based programmes.
Provide support to patients within group sessions as required.
Be responsible for taking and accurately recording measurements within a group or clinic setting, e.g. weight, height, and blood pressure.
Implement care plans agreed by Cardiac Rehabilitation team including assisting patients in physical exercise and risk factor reduction advice.
Contact those who did not attend group sessions to attempt to facilitate re-engagement.
Be responsible for team resources, including maintaining stock levels of resources and managing the regular servicing of electronic equipment.
Facilitate the smooth running of both telephone, outpatients and face-to-face assessment clinics by greeting patients, co-ordinating questionnaire completion and undertaking baseline measurements.
Assist CR Team to educate patients, carers, and families as appropriate.
Act as a role model, valuing everyone's opinions, values, and experiences.
This role is based in the community with visits to the hospital/community leisure centres and home environments as required.
