Job summary
The post holder will work as part of the Community Physio team primarily in Torquay but may at times assist with the Occupational therapy, Intermediate Care/Duty teams in the zone or Baywide when needed. Delivering health care that focuses on the direct need of service users, within the home environment or appropriate care setting throughout Torbay.
They will carry out specific clinical tasks and responsibilities as delegated by registered practitioners.
The post holder will be part of an MDT, working with and supporting all members of that team both within and external to the directorate.
Supervision and teaching of junior staff will be part of the role.
These will be identified and agreed at interview and yearly appraisal.
These additional responsibilities may change on an annual basis and work towards increasing the clinical competencies of the individual.
Main duties of the job
- To work independently and autonomously, providing assessment, support, care, treatment and advice to individuals, within guidelines and protocols identified for the role.
- To work within a multi-professional team undertaking practitioner role.
- To implement treatment and care to patients under the guidance and supervision of a registered professional. e.g. Mobility, Personal Care, Equipment Provision
- Wherever possible to contribute in reducing inpatient length of stay to ensure that patients receive the appropriate care in the most appropriate setting.
- To keep the patient at the centre of their care, ensuring patient and carer participation in decision making.
- To take responsibility for the daily care and support of delegated patients. To support qualified staff in maintaining Best Practice within fractured neck of femur and Rapid Recovery pathways to include support with data collection .
- Deliver treatment/care within a variety of settings, under the guidance of a registered practitioner.
- To practice the principles and philosophy of individualised person centred patient care in accordance with Trust Policy, procedures and regulatory body's guidelines
- To participate in regular weekend service rota.
