Job summary

The post(s) will be ward-based on Templer and Teign wards at Newton Abbot Community Hospital. Teign Ward is a 30 bedded ward that aids in the rehabilitation and ongoing medical care of patients before they are suitable for discharge. The majority of these patients are frail and have multiple co-morbidities. A consultant physician will be providing a weekly ward round on Teign.

Templer ward has 20 stroke and neurological rehabilitation beds and 10 general medical beds and the specialist doctor attached to this ward is also supported by a weekly ward round with the Stroke consultant.

The posts would be to provide a 52 week per year level of cover for these beds alongside three other senior and experienced doctors.

This role will give an opportunity to become more independent in the care of frail older people, Comprehensive Geriatric Assessment (CGA), and allow more autonomous decision making.

There would be no on-call commitment within these roles, however there is the expectation that there will be some bank holiday working to ensure the ward is not without medical support for longer than 3 days.

Whilst this role is based at Newton Abbot Hospital there may be the requirement in extremis to work at Totnes Community Hospital. This would be agreed first through the medical lead for NAH.

This post would suit anyone interested in Geriatric, general or community based medicine including those wishing to pursue their GP training.

Main duties of the job

  • Providing a clerking service for patients that are newly admitted to the wardand commence a comprehensive geriatric assessment. Patients could come from Torbay, Exeter or Plymouth.
  • Review patients that are having active medical care, and those that havebeen highlighted by the nursing team.
  • Liaise between nurses, patients, relatives, and senior medical staff.
  • Participate in the daily MDT meetings for inpatients.
  • Completion of discharge summaries
  • Supervise medical students where appropriate
  • Co-operate with members of the personnel department when monitoringhours of work and other personnel issues
  • Comply with all local policies including dress code, annual and study leave

Job description

Job responsibilities

The working day will start from 9.00 and finish at approximately 5pm. You would start by identifying the nurse in charge and attending the daily safety brief, which will highlight any Patients who need an urgent review, are new admissions to the ward or need any medical intervention prior to discharge that day. You will have full access to all relevant clinical systems to be able to request and review any tests.

There are on site x-ray and ultrasound facilities within the hospital from 9-5 which you can refer patients to.

There is currently a courier service three times per day (0800, 1300 and 1530 and ) that will take blood samples back for analysis at Torbay hospital. There is no phlebotomoy service on the wards so on occasion you may have to take blood samples yourself.

There is a daily SAFER meeting at 1300hrs that you will be required to attend where each patients EDD and goals for discharge will be discussed and agreed. TTAs should be requested 48hrs prior to discharge where possible to avoid any delays and Discharge summaries completed prior to discharge.

There will be at lease one GP or SAS grade Doctor on site daily and a weekly Ward round speciality consultant.
