Job summary

Torbay and South Devon healthcare trust are looking for a Healthcare Support Worker to work as part of their multidisciplinary team, providing high quality diabetes care to the local population within the integrated care organisation. The role is working under the supervision and assisting the diabetes specialist nurses on a day to day basis as well as working alongside other members of the multi-disciplinary team.

We are looking for an individual with good organisational and communication skills, and the ability to both work under their own initiative and follow directions. As technology for glucose monitoring and insulin administration has evolved, we are looking for a member to join the team to be involved with supporting patients and to provide pre-planned diabetes education to individuals and carers.

Our ideal candidate will be forward-thinking, motivated, and enthusiastic about working with people and providing personalised care. Its aim is to improve health outcomes for people with diabetes, both by providing interventions and by increasing the level of skills and knowledge within primary and secondary care.

The goal of the service is to enable patients to access education, information, advice and support to help them to make informed choices about how to manage their condition. Joint care planning and goal setting is a core focus, with patients being empowered to self-manage their health. This permanent vacancy is part-time, to be worked over a five day working week

Main duties of the job

To work as a member of the Diabetes multidisciplinary team to improve the standards of care for patients with diabetes by providing knowledge and skills as defined by national competencies

To contribute towards the training and education of patients, carers and healthcare professionals within primary and secondary care settings

Undertake inpatient, outpatient and community-based assessment and education under the supervision and delegation of the diabetes specialist nurse team

To carry out assigned tasks and duties that support and compliment the work of diabetes specialist nurses.

To support the diabetes multidisciplinary team in providing services

Job description

Job responsibilities

Communicate effectively with patients, families/carers, recognising there may be a need to utilise alternative methods of communication. Where there are barriers to understanding, as required arrange access to information, support and other services to aid effective communication

Liaise daily with the diabetes specialist nurses regarding inpatients and outpatients diabetes care plans

Undertake telephone reviews for patients under the care of the team using agreed pro-forma to record the review and planned actions

Provide training/presentation sessions for other healthcare professions and patient and their carers as delegated by the diabetes specialist nurse

Participate in the initial assessment of patient needs under the direct supervision of a registered practitioner

Support the preparation of multidisciplinary clinics by ensuring all prespecified

investigations and downloading of insulin pumps and glycaemic monitoring devices are completed prior to clinic

Order insulin pump consumables or diabetes technology as directed by the registered nurse

Support the discharge planning of patients with diabetes under the guidance of the diabetes specialist nurses

Undertake a range of delegated and specific clinical skills for patients that demonstrate an enhanced level of competency /technical expertise documented in the plan of care

Recognise when a patients condition changes, identify patient related concerns and seek advice and expertise from registered practitioners in a timely manner

Teach patients how to perform blood glucose and blood ketone monitoring along with quality control checks and report findings to a registered nurse

Teach patients how to perform continuous glucose monitoring and report findings to a registered nurse

Develop resources for displays at study days/for health promotion

Participate in the national diabetes audits
