Provision of leadership to the Intermediate Care team within Torbay
To be part of the locality's management team, providing leadership to the wider, locality-based Health & Wellbeing Team
To contribute to the strategic development of Torbay Locality, in partnership with the Deputy Community Service Manager, Community Service Manager and other Professional Leads
To contribute to the strategic development of Intermediate Care in partnership with wider Community Service Managers/other Locality Leads
To work as a supportive and inspirational clinical leader within the integrated Locality team, providing a high standard of service-specific expertise
To work closely with the primary care networks to ensure that Intermediate Care services are delivered in a safe and effective way; with the outcome of providing an alternative to hospital admission, enabling earlier hospital discharge and preventing early admission to long term care
To develop and maintain professional working relationships with the Care Homes commissioned to provide Intermediate Care bed-based care

About us
Intermediate Care supports the acutely unwell people of Torbay, to manage as safely and as independently as possible in their own homes. Intermediate Care has three main goals:
- Preventing unnecessary hospital admissions
- Facilitating timely hospital discharges
- Preventing the need for long term care packages and care home placements, where possible
We pride ourselves on being person centred and flexible, providing high quality, short term interventions.
