Job summary

We have an exciting opportunity Under the supervision of a Band 6, Occupational Therapist you will work as a member of a multi-disciplinary team in short term services (reablement) providing specialist occupational therapy input aimed at achieving maximum independence for all service users.

Main duties of the job

  • Carry out holistic assessments and provide the multi-disciplinary team and family with recommendations regarding appropriate level of support/care, adaptive equipment, and minor adaptations
  • Following a holistic assessment in the patient or service user's own home, provide recommendations to help maintain the individual at home; to include appropriate level of support/ care; adaptive equipment and minor adaptations
  • Carry out specialist assessments such as cognitive, perceptual, wheelchair assessments, carer's assessments, and Multi-Factorial Falls Assessments
  • Education/teaching patient functional tasks safely; advice regarding specialist techniques such as energy conservation; joint protection and work simplification
  • Advice and support carers on aspects of care and the promotion of health to maintain care in the community for as long as possible

Communication and working relationships

Ensure effective communication systems are maintained between

o Multi-disciplinary or Health and Social Care Team to provide consistent, well-co-ordinated patient/service user care

o Relatives and carers to discuss patients condition and associated problems as appropriate to confidentiality

o Relevant Social Services departments, statutory and voluntary agencies involved in patients care, as appropriate to confidentiality and Date Protection Act

Communicating sensitive information regarding loss of independence and patients health and social welfare

Communication with patients who may have sensory or cognitive impairments, perceptual deficits, or language problems e.g. patients with dementia or stroke

Analytical and judgement

Evaluate care given and amend the occupational therapy process as necessary to ensure maximum effectiveness of treatment; utilise outcome measures appropriate to clinical / work area

Planning and organisation

Working in accordance with the professional standards for the service, assess the patient/service users needs and work with them to plan and implement an Occupational Therapy programme to provide the highest standard of patient care

Following a holistic assessment to provide the patient/service user, multi-disciplinary team, and family with recommendations regarding discharge from hospital to include appropriate level of support/care, adaptive equipment, and minor adaptations

Plan and manage your own caseload

Physical skills

Carry out manual handling risk assessments; advise on appropriate handling techniques; assess and secure provision of appropriate manual handling equipment including hoists

Have own car and people to drive though out south devon.

Patient and client care

Carry out holistic assessments and provide the multi-disciplinary team and family with recommendations regarding discharge from hospital or residential placement; to include appropriate level of support/care, adaptive equipment, and minor adaptations

Following a holistic assessment in the patient or service users own home, provide recommendations to help maintain the individual at home; to include appropriate level of support/ care; adaptive equipment and minor adaptations

Promote an understanding of the role of the Occupational Therapist within your areas of work and assist in the provision of training to Occupational Therapy Technical Instructor/ Assistant, students and other relevant personnel in order to facilitate effective multi-disciplinary work

Provide clinical support and supervision to support staff.

Mentor or supervise students on clinical placement

Participate and comply with mandatory in-service training

Teach support staff techniques to care for the patient, including use of adaptive equipment and manual handling techniques

Information technology and administrative duties

Participate in clinical audit as required for clinical governance purposes

Comply with statistical, report writing, documentation and recording requirements as specified in the standards in order to demonstrate the provision of an efficient and effective quality service; this to include ICS, Paris, care first 6 and access suite as appropriate to area of work

Research and development

Ensure that own professional knowledge is constantly updated in order to maintain a high standard of patient/client care; maintain a current CPD portfolio

Participate in supervision and yearly review process in order to achieve personal development and a high standard of patient care
