Job summary

To work as a Healthcare Assistant within an integrated team, working across the community, this includes: Community Nursing, Intermediate Care and Community Matrons.

Main duties of the job

To deliver high quality care to the housebound patients of Torquay and work as part of multi-disciplinary team and in partnership with primary care and secondary care.

Job description

Job responsibilities

  • An ability to effectively communicate routine information to patients/carers where co-operation is required and where there may be barriers to understanding
  • Assisting trained nurses in delivering a high standard of holistic nursing care to patients in the home environment, within the dimensions of the post
  • Implementing delegated, planned and appropriate aspects of nursing and social care unsupervised and being responsible for informing the Community Nurse Team Leader/Nurse in charge of observations relating to patient care

Qualifications and training

Essential

  • NVQ level 2 with evidence of working towards level 3

Desirable

  • NVQ level 3 in Health

Knowledge and experience

  • Basic IT skills
  • Excellent communication and interpersonal skills
  • Ability to maintain accurate written records

  • Broad range of simple nursing procedures eg. Venepuncture, BP monitoring, basic wound care.

Specific Skills

  • The ability to liaise efficiently and effectively with patients/carers, a wide range of health and social care workers GP's and formal and voluntary care providers within the primary care setting

  • Experience of working within the primary care setting

  • The ability to work independently in a potentially unpredictable and occasionally highly unpleasant working environment; to include body fluids and working in extreme weather conditions
