We are looking to recruit a Health and Wellbeing Coach, to work within our primary care network multidisciplinary healthcare team, providing 1:1 and group personalised care support and health promotion to people who are referred to them by team members and local agencies.
The successful candidate will have good communication, negotiation and listening skills and act with compassion and integrity. They will have experience of using coaching approaches, frameworks and models or other helping strategies, for example, motivational interviewing and a nutritional degree.
The post holder will work with a diverse range of people from different cultural and social backgrounds. The ability to work confidently and effectively in a varied, and sometimes challenging environment is essential.
This is a vital role within the primary care network and the post holder will provide health coaching to people with long term conditions to support them to increase their self-efficacy, motivation and commitment to change their lifestyle, improve health, and manage symptoms. They will work closely with GP and practice teams to manage a caseload, ensuring appropriate support is available to people, families and carers.
Main duties of the job
Health and wellbeing coaches support people to take proactive steps to improve the way they manage their physical and mental health conditions, based on what matters to them. They support people to develop their knowledge, skills, confidence in managing their health and care, to improve their health outcomes and quality of life and support them making changes in health related behaviours. Health coaches do this by coaching and motivating people through multiple sessions to identify their needs and to set goals for themselves.
Health and wellbeing coaches manage and prioritise a caseload, according to the needs, priorities and support required by individuals in the caseload. They identify when a person's needs are beyond the scope of the health and wellbeing coach role and refer them back to other health professionals or organisations. At the Coastal Network the Health and wellbeing coach may work with people by phone, by video conference, or face-to-face - on a one to one basis and in groups.
The successful candidate will be compassionate, reflective and self-aware and will enjoy working with a wide range of people. They will have good communication and negotiation skills and a firm belief that people have untapped resources within them, that can be developed by providing high quality, non-judgemental support.
About us
At The Coastal Network the health and wellbeing coach will be a key part of the PCN multidisciplinary team. They will work closely alongside a team of Social Prescribers to provide an all-encompassing approach to personalised care and promote and embed the personalised care approach across the PCN. The Health and wellbeing coach role will deliver group sessions up to one evening a week and support the design of a proactive health promotion scheme which will benefit the local communities within Dawlish and Teignmouth. Therefore someone with a nutritional qualification and experience of group delivery and outreach work would be advantageous.
