The post holder will work as an experienced nurse supporting the identification, assessment, management and treatment of complex older adults who have attended the Torbay Hospital emergency department or one of our short stay admission wards as part of a Multi-Disciplinary Team to help avoid hospital admission.

Main duties of the job

The post holder will support frailty identification and provide relevant interventions while taking a home first approach and helping to avoid hospital admission.

The post holder will work alongside other members of the multi-disciplinary team, emergency medical team, frailty team, allied health professionals and social care colleagues. They will also liaise directly with community services to ensure smooth onward care.

About us

The Joint Emergency Team (JET) are a Multi-Disciplinary Team consisting of Nurses, Occupational Therapists, Physiotherapists and Discharge Co-ordinators.

They are based within the Torbay Hospital Emergency Department and work alongside the Emergency Department team having shared responsibility for patients identified for JET service intervention.

The JET team also work within our Medical Receiving Unit (MRU) or other relevant ward areas. The JET service is provided over 7 days from 8am to 6pm.

JET members need to be able to work well within a Multi-disciplinary team but able to work independently and have excellent communication skills.
