An exciting opportunity has arisen, this opportunity allows you to undertake holistic needs-led assessments in partnership with service users, patients and carers.

You will manage a personal caseload of complex, highly complex and statutory work, and act as an autonomous practitioner evidencing high levels of problem solving, decision making and independent judgement in a variety of community settings and situations.

We work in an integrated way with allied health care professionals within the locality, and are a motivated and caring team to both patients and colleagues therefore, your role with us willinvolve being part of the Urgent Care Response team, led by a band 7 OT , responding to the same day needs in the community, including hospital discharges. The aim is to manage any crisis before referring on to the appropriate community teams, which the Urgent care team work very closely with. These teams include Intermediate Care and Community Therapy.

To effectively undertake the role, you will be required to work flexibly to ensure a consistent and robust service, which covers seven day working including weekend and bank holiday work, or working in other localities as necessary to meet the needs of the service.

To explore innovative ways of collaborative working between general practice, health and social services and the private and voluntary sector in line with the Care Trust's integration agenda.

Main duties of the job

  • To work as a multi-disciplinary team contributing to joint decisions regarding the management of patients, including acutely ill patients referred by the GP or A&E/ED
  • To make a difference and help patients through their recover and returning back to health and independence.
  • The post-holder may have supervisory responsibility for a Band 5 post or Assistant Practitioner as appropriate.
  • The post-holder has responsibility for managing a community and IC caseload.
  • The post-holder will assess and treat patients in a variety of community settings; patient's own home, nursing homes, residential homes.
