Job summary

A great opportunity to join the Bristol Royal Infirmary Frailty Team.

The current model of working is from the Front door, ED and frailty Sdec in order to avoid admission and facilitate discharge whilst continuing to build key stakeholder relationships to further improve patient pathways, outcomes and experiences.

As a qualified AP having completed rigorous training It is expected that you have achieved an MSC in Advanced Clinical Practice and be able to provide evidence of a robust portfolio against the specialty COTE capabilities in practice. You will perform advanced assessments of the frail older patient with diverse presentations, complex physical and psychological conditions, utilising a comprehensive geriatric assessment, to determine a diagnosis, formulate and deliver an appropriate, individualised treatment programme. You will have expert working knowledge of community services for admission avoidance and safe discharges.

Main duties of the job

To undertake a comprehensive, holistic assessment of patients including those with highly complex presentations/multi pathologies, using highly advanced clinical reasoning, analytical and investigative skills and highly developed specialist manual assessment techniques to provide an accurate diagnosis.

Have an expert working knowledge of the comprehensive geriatric assessment.

Actively lead the development of care planning and admission avoidance initiatives through advanced clinical skills.

Lead the development of clinically effective, multidisciplinary team of practitioners to enable the implementation of effective admission avoidance initiatives

Support/ lead on development of integrated working an integrated pathways with Primary and Secondary care colleagues and frailty services

Be responsible for the development and delivery of a consistently high quality service, promoting frailty care as a positive place for staff experience and development.

Act as an expert practitioner and a role model to and on behalf of the whole team, to ensure greater integration of the frailty pathways into health & social care admission avoidance initiatives.
