Job summary

This exciting permanent, Band 6 opportunity offers the applicant the chance to join the Community Children's Nursing (CCN) Team to provide care for children and young people aged 0-16 with a nursing need, in their home or community setting.

We are seeking experienced paediatric nurses who are reliable and forward thinking with excellent clinical, organisational and interpersonal skills to join the team.

The CCN Team aim to promote early discharge from hospital and to prevent, where possible, planned or emergency admissions to hospital. Common referrals seen by the team include: administration of intravenous antibiotics (p-OPAT service), injections, wound care, post-surgical care, oxygen dependence and clinical assessment / review. The CCN Team also work very closely with the Avon and Wiltshire Mental health Partnership to deliver a highly innovative programme of support for young people requiring feeding support in the community.

The applicant must have recent and varied paediatric nursing experience, be able to work autonomously and work well within a small team. Car driver and access to own vehicle is essential.

The community service operates 7 days a week between the Core service hours of 08.00-1800 including bank holidays and weekends, covering the geographical area of Bristol, South Gloucestershire and North Somerset.

Main duties of the job

Prioritise and take clinical responsibility for a delegated patient caseload, practising with minimal supervision. Assess, plan, implement and evaluate care for Children with a Nursing need in their own home or at school.

Deliver skilled, evidence based care, support, advice at home, as well as ensuring that appropriate resources are available to assist in providing this care.

Act as a resource of knowledge and expertise - in all components of community nursing practice to ensure optimum and appropriate care delivery to children and their families/carers.

Provide child/young people and their families with appropriate written information/ advice.

Ensure nursing interventions at home and telephone advice are accurately documented

Keep up to date with all policies/procedures including local and national practice guidelines ensuring that a high standard of care is maintained. This will include participating in the development of new protocols/ standards as well as supporting change initiatives developed in response to any identified need(s).

Liaise appropriately with other members of the multidisciplinary team and refer to external agencies as appropriate.
