Job summary

We are excited to be expanding our Advanced Practitioner (AP) workforce across (Medical Assessment Unit) MAU, Same Day Emergency Care (SDEC), Geriatric Emergency Medicine (GEMs), Older Persons Assessment Unit (OPAU) and our Emergency Department (ED) to support the planned changes to Urgent Care at Weston General Hospital due to Healthy Weston 2

Healthy Weston 2 outlines our Vision to support development of a service where patients are seen and treated in the right place and time and are discharged home without unnecessary delays. A great amount of this work will take place at the front door within our Urgent Care.

Programme aims to see Weston General Hospital as a centre of excellence for frailty and elective surgery within the wider UHBW Trust

Applicants applying for the qualified posts must hold an MSc in Advanced Practice (or equivalent award). For the ED post you must also have evidence of achieving or working towards Royal College of Emergency Medicine (RCEM) credentialing. The post holders will be registered as an Independent Prescriber

Interested applicants for all posts must have significant experience at a senior level working within an urgent care setting and have experience to support the ability to see and treat patients autonomously within the desired environment. We require enthusiastic, forward thinking, well organised and motivated individuals with a positive attitude to join our multi-professional teams to work within Urgent Care at Weston General Hospital

Main duties of the job

The Advanced Practitioner (AP) will use specialist knowledge and skills to providehealthcare independently to patients. The AP is accountable for the total episode of care for patients with undifferentiated and undiagnosed needs.

The post holder will act as a role model, delivering advanced practice and promoting clinical effectiveness in care.Theywill be requiredto work autonomously in delivering independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment, diagnosis, prescribing, health promotion and safe discharge of patients.
