Job summary

At UHBW we are creating two new Transfer of Care Hubs to support our patients with discharge planning from admission through leaving hospital and into the community; one Hub at the Bristol Site and one at Weston General Hospital. The Transfer of Care Hub will bring together, Acute Trust Case Managers, and Patient Flow Coordinators, Social Workers, Local Authority Discharge Coordinators, Sirona Case Managers, Community OT's, the Homeless Team, Voluntary Services and Discharge and Flow Coordinators.

This is a challenging and interesting role to support both Transfer of Care Hubs (WGH and BRI) and we are looking for an exceptional individual who wants to consolidate their skills and experience. Key requirements for the role are: excellent interpersonal skills; significant experience of NHS operational management; a good understanding of the broader NHS system and political environment; service improvement and change management experience and a passion for high quality, compassionate care.

The successful candidate will be organised, self-motivated and resilient. They will need to have excellent understanding of performance management and significant experience of delivering objectives in a challenging health care environment.

The role operates within a complex and challenging environment and we are seeking an exceptional individual with a broad range of skills and experience to work closely with the Assistant Director of Operations.

Main duties of the job

  • To line manage the Improvement lead, Operations Manager and Data and Operations Manager and the administrative function of the IDS.
  • Contribute to strategic planning of the service
  • To provide day-to-day leadership of line-managed staff, such as sickness leave, performance, capacity and demand management.
  • To be up to date with guidelines and practise and apply a planned approach to the discharge of complex patients ensuring a comprehensive, integrated and efficient service is provided.
  • Develop a standardised administrative approach across both sites, establishing cross-site oversight, governance and quality assurance for the ADO.
  • To lead on the development of skills and knowledge across the non-clinical team, supervise and mentor staff as required.
