Job summary
This is an exciting split post which involves working within the pharmacy anticoagulation team as well as being the pharmacy link for facilitating discharges from hospital.
The UHBW anticoagulation service is run by pharmacy professionals with the purpose of optimising anticoagulant therapy for patients across Bristol. This is an excellent opportunity for you to gain in-depth experience dealing with a wide variety of patients and health care professionals in order to safely manage patients with their anticoagulant medications. The service is expanding to include an inpatient referral service where the anticoagulation team can support staff members with complex anticoagulation management for patients within the hospital.
You will be the first point of pharmacy contact for the Hospital Discharge Team & BRI Discharge Lounge staff.This is a new role so it will be flexible; you will be able to make it your own and make improvements as you go along. The aim is to help facilitate safe and timely discharges by resolving any medication related issues, working with the dispensary team to assist with prioritisation and work flow, and liaising with teams within the hospital and in the community.

Main duties of the job
Anticoagulation services: Determine safe and appropriate dose of anticoagulants for referred patients to maintain an optimal level of anticoagulation to minimise adverse effects: Take into account concurrent drug therapy, diet and alcohol consumption, lifestyle, pregnancy status and social conditions in management of anticoagulants.Refer unstable or uncontrolled oral anticoagulant patients to other healthcare professionals as appropriate: Accept referrals to anticoagulant service, create and maintain the relevant written and computerised patient records. Counsel patients, providing specialist advice, instructions and reassurance regarding safe and appropriate use of oral anticoagulation: Provide advice on warfarin, DOAC, and heparin dosing switching between various anticoagulants to prescribers.
Discharge Services: Review daily pathway discharge profile: Facilitate resolution of pharmacy-related issues to prevent delays to patient discharge: Assist Pharmacy & Hospital Discharge teams with timely discharge of patients with complex medication. Work closely with dispensary co-ordinators to facilitate prioritisation in dispensary: Liaise with Integrated Care System organisations as appropriate, to facilitate safe and timely patient discharge. Assist patients and carers in Discharge Lounge with medication counselling and complex medication-related issues arising during discharge process: Refer complex patients to Discharge Medication Service: Attend relevant meetings.
