Band 6 Cardiac Rehab Nurse or Physiotherapist
The University Hospitals Bristol and Weston NHS Foundation Trust (UHBW) cardiac rehabilitation offers a hybrid service including Face to face and remote telephone-based rehab. Cardiac Rehab is one of NHS England's top 5 priorities and we are a growing service supporting patients post cardiac surgery, Myocardial infraction, Percutaneous Coronary Intervention (PCI) and Heart failure. The team is innovative and was nominated for the UHBW Recognising Success Awards for the Clinical team of the year 2023. The UHBW cardiac rehabilitation offers a hybrid service including Face to face and remote telephone-based rehab as well as early rehab (education in inpatient settings) and community cardiac rehab classes in leisure centres.
Main duties of the job
The post holder will assess patients for cardiac rehabilitation and will manage the patients cardiac rehab journey throughout the face to face or remote Cardiac rehab pathway. This role will enable the opportunity to develop leadership skills in a multidisciplinary environment with support and development of registered staff. You will also have the opportunity to develop project skills and help shape Cardiac rehab at UHBW including participating in a heart failure pilot and feeding into the development of cardiac rehab guides.

- Experience of working as part of a Nursing/Physiotherapy team and/ or multidisciplinary team
