Job summary
An exciting opportunity has arisen for a Band 7 Practitioner in Dementia, Delirium and Falls based in Weston Division at University Hospitals Bristol and Weston NHS Foundation Trust (UHBW).
This is a strategic role in ensuring and consistently developing high quality care for people with dementia, delirium & falls across the Trust, consistent with the principles and objectives contained within the National Strategies. To ensure that the services provided are responsive to patient needs, that the planning of services involves patients and users, and reflects the diverse population served.
As a specialist practitioner you should be able to demonstrate your experience, knowledge and skills in relation to dementia care. You will assist in identifying people and their carers living with dementia and provide support where there are behavioural changes.
You will need to use observational and comprehensive assessment skills to, advise, record a plan of care and assist the transition towards a safe discharge. You will require presentation, teaching and auditing skills as the role is diverse.

Main duties of the job
Patient Care:
- To provide specialist advice and support to patients, their families and carers from the time of diagnosis and throughout treatment and follow-up phases of treatment and care.
- To lead the development, improvement, management and promotion of the service ensuring the delivery of high quality, cost effective care.
- To lead on the implementation and monitoring of adherence to relevant clinical guidelines.
- To provide all staff with specialist advice on the management of symptoms, de-escalation advice, falls prevention & management.
- To educate and share expertise with the multidisciplinary teams
- To act as a resource in the care of patients with specialist care need
