Job summary

Sister/Charge Nurse - Band 6

Are you looking for a new challenge managing acutely unwell adults in their own homes? If you want to work with a range of doctors and other healthcare professionals for this brand new Hospital at Home service wed like to hear from you.

We are looking to recruit a number of Band 6 nurses.

The service will give some patients the opportunity to receive acute care and treatment in their own home. Patients will be admitted on a case-by-case basis.

Main duties of the job

We are looking for nurses with Advanced Clinical Skills and those willing to develop further to be part of Hospital at Home Team. If you are keen to progress in your career in this exciting new service this could be the role for you. You will have the full support of the clinical team.

Please note a valid driving license and access to a vehicle is essential for this role.

Job responsibilities

The post holder will deliver patient care on a daily basis. They should be expert in all areas of nursing care from a patients admission to discharge on the Hospital at Home pathway. As senior nurse, they have overall responsibility for day-day management of their clinical caseload and ensure all care is delivered according to Trust policies and procedures associated records. They will manage and support a team of nurses and allied professionals in the delivery of evidence based, patient centred care, which is high standard. . They will assist in the management of staff appraisal, rosters, recruitment and the supervision of learners. They will provide advice and support to patients, relatives and members of the multidisciplinary team. Utilising clinical audit, research, supervision of practice and teaching, they will monitor and improve standards of care by efficient and effective use of resources and liaise with the senior sister at all times. The will provide leadership on the Trusts values and behaviours.

To assist with patients boarded on the Hospital at Home pathway; working with acute frailty services, GPs, Community Matrons, District Nurses, Therapists and Rehabilitation Assistants. The patients may already be known to Adult Community Services who have had a change in their care/nursing needs or new patients referred in from external services. The role involves visits to patient homes to ensure they receive the interventions and care required to enable patients remains at home or their usual residence in a safe and supported environment. This care reviewed daily by the MDT and referred onto the most appropriate service/services within Adult Community Services and/or external organisation.

The service operates 8am to 8pm 7 days a week and the post holder is required to work shifts within this times. The Hospital at Home Team are based in the Community Co Ordination Centre.

Qualifications

Essential

  • NMC registered Nursing Qualification & evidence of Continued Professional Development (CPD).

Desirable

  • Physical Assessment at Level 6 or 7 or a desire to complete this as CPD.
  • Appropriate Teaching Qualification.
  • First degree.

Experience

Essential

  • Understanding in differential Diagnosis / Physical assessment in order to formulate an impression as to why a patient may have deteriorated.
  • Experience at Band 5 or above.

  • Current knowledge and understanding in Adult community services.
