Job summary
Surrey and Borders is looking for someone who is passionate about ensuring a high quality holistic approach to the care and support provided to clients in the community who have been identified as in need of community nurse support to enable them to exercise choice over their own lives.
Job Overview
- To assess, plan, implement and evaluate the nursing care needs of a specific group of people who use our services.
- To mentor, supervise and teach qualified and unqualified staff including students
- To lead the care for a specified group of service users.
- To participate in the recruitment, deployment and management of staff.
Main duties of the job
- To provide high-quality, holistic, person-centred care to service users in the community.
- To lead the clinical management of a defined caseload, ensuring care is effectively planned, implemented, and evaluated.
- To coordinate and deliver evidence-based interventions, including CBT-informed therapeutic support.
- To work collaboratively within the multidisciplinary team (MDT) and with external agencies.
- To undertake comprehensive assessments, including risk assessments, and respond appropriately to changes in clinical presentation.
- To monitor and support both the physical and mental health needs of service users, including undertaking clinical observations.
- To promote recovery, independence, and choice, enabling service users to exercise control over their lives.
- To ensure safeguarding responsibilities are met and risks are managed in line with policies and procedures.
- To maintain accurate, timely, and legally compliant clinical records.
- To communicate effectively with service users, carers, families, and professionals.
- To contribute to service development, quality improvement, and audit processes.
- To support and contribute to staff development, supervision, and learning.
- To ensure compliance with professional standards, including the NMC Code, and Trust policies.
- To promote a positive image of the team and organisation at all times.

This role is working within i-access, a successful, countywide drug and alcohol treatment service for Surrey residents delivering tier 2, 3 and 4 substance misuse interventions across 3 main bases and 25 satellite locations. i-access is run collaboratively by SABP, PH / SCC and Via under a cooperative agreement which is one of partnership and transparency and contributes to Surrey Heartlands wider strategic ambitions.
