We are looking for a Urology Support Worker to join our specialist team for a 12 month maternity cover. Under the guidance of our Urology Clinical Nurse Specialists (CNS) and Specialty Manager, you will play a vital role in supporting our teams to deliver an effective service to our patients during and after treatment. You will be the bridge between the patient and the clinic, ensuring no one feels lost in the systeAre m--particularly those on urgent Two Week Wait (TWW) pathways for suspected betladder cancer and urodynamic investigations. Main duties of the job
Coordinate care by providing a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needs, especially patients on a TWW pathway for suspected bladder cancer.
Coordinate care and education and support for people with non-complex care needs (Level One - Supported self-management -(National Cancer Survivorship Initiative NCSI risk stratification model)
