We are searching for a dynamic and adaptable therapist to support individuals in achieving their functional goals at home following their discharge from hospital.

Main duties of the job
You will work alongside a supportive and skilled team of therapist, managing your own caseload while delivering outstanding patient care. This is a chance to make a real difference in people's lives by providing flexible, forward-thinking support across a range of clinical specialties.
This role includes some weekend and bank holiday working.

Managing complex assessment, with predominantly physical conditions and with regard to the physical, emotional, psychosocial, spiritual and social aspects of their care.
Directly responsible for the management of own caseload of patients with diverse presentation and complex physical and psychological conditions, referred for Occupational Therapy assessment, rehabilitation, treatment planning and discharge management.

Essential
- Diploma/degree in OT by recognised awarding body e.g. Royal College of Occupational therapists.
- Must satisfy the criteria for full membership of the Health and Care Professions Council (HCPC).
- Further qualifications/study
- Driving licence (and use of own car for work purposes).
Desirable
- An approved Occupational Therapy supervisor for OT/PT degree students
Knowledge
Essential
- Knowledge of the role of Occupational Therapy for people with a wide range of physical dysfunction
- Previous experience relating to specialist area.
- Experience of working in an acute hospital setting.
- Knowledge of relevant NHS and SS legislation.
- Experience with community/mental health conditions.
- Used to working in a busy environment
- Working knowledge of Microsoft Office and any other relevant It systems applicable to the applicant.
- Experience of undertaking clinical supervision.
- Experience of managing and motivating a team and reviewing performance of the individuals.
- Thorough knowledge of the Inclusion (equality and diversity) agenda.
