Do you enjoy working in an exciting and challenging environment? We are looking for a part time Supportive and Palliative Care Coordinator to join us in Royal Surrey Cancer Centre, Guildford. This is a new role and offers an exciting opportunity for someone to join our friendly and dynamic team.We are looking for an individual who can demonstrate excellent communication skills, good IT skills and ideally a working knowledge and experience of working within a patient focused role. NHS experience is desired but not essential.The successful applicant will work alongside another SPCT coordinator and be able to demonstrate that they can work upon their own initiative whilst working well within a team. The role will include coordinating care of patients with life limiting conditions, and offering support under the supervision of the SPCT clinical nurse specialists. The role will require communication with all members of the SPCT and wider MDTs. Being the first point of contact for the team, patient queries will be an essential part of the role and will therefore require an empathetic and can do attitude. This is a great opportunity to combine an administrative post with some patient facing activities. Main duties of the job

Under the direction, guidance and supervision of the Supportive and Palliative Care Clinical Nurse Specialists (CNSs) and Enhanced Supportive Care project manager, the post holder will help coordinate the care, and offer support to patients with life limiting conditions. They will perform administrative duties for the team in order to ensure the smooth running of the service.

Coordinate care and support people with life limiting conditions who are under the care of the supportive and palliative care team. Perform administrative duties to enhance the delivery of an efficient service.

To have made an effective contribution to reaching the Trust's vision, strategic objectives and key work programmes.

Job description

Job responsibilities

Assist in the coordination and booking of appointments and assessments as required including SPCT ward follow ups and new Enhanced Supportive Care referrals to the team.

Record outcomes of symptom assessment patient questionnaires on behalf of the SPCT OP team and ensure they are correctly uploaded onto the EPR

Assist in the collection and recording of outpatient and inpatient activity data required for defined SPCT Key Performance Indicators (KPIs).

Able to work autonomously. Refer to Palliative Care CNS when necessary.

Monitor and record patients put onto Patient Initiated Follow Up (PIFU) list following divisional protocol

Support and contribute to audit processes, governance, research, clinical research trials and service development as appropriate

Comment on proposed changes to policies and procedures

Keep Datix and Complaints log up to date and help disseminate any learning to the team as appropriate

Act as a facilitator to resolve issues that may be perceived as barriers to care, for instance making sure patients are seen promptly and in the correct clinical setting.

Planning/inviting/organising of Health and Well-Being events or other supported self- management events with other support workers

Identify personal education needs and skills development. Demonstrate self-directed learning, actively seeking role development opportunities to enhance practice, knowledge and role progression

To provide cross cover for SPCT administration staff and project manager as required.

Order supplies and equipment required by the team

Monitor incoming calls to the Supportive and Palliative Care Team (SPCT) and initiate appropriate response using good communication skills, basic clinical awareness and appropriate tools and procedures, liaising with the team for potential escalation as appropriate

Understand that there will be exposure to patients and relatives who may be very distressed, and provide a compassionate and reasonable response at all times.

Communicate and signpost to appropriate needs related information. Ensure all patient leaflets are distributed (eg. To patients and families of patients being cared for on the end of life care plan) and up to date as required. Ensure information relating to Enhanced Supportive Care and Supportive and Palliative Care is correct on Royal Surrey Cancer website

Coordinate the handover documentation with other community palliative care teams to facilitate safe and effective transition of care between services in order to provide seamless support for people

Liaise with other teams within the hospital to help foster good working relationships and shared care of patients

Make sure that people referred to the team are aware that they are interacting with a SPCT professional and know about the full range of resources and services available.

Ensure the DragonFly boxes are kept stocked up on the wards, offer items to patients and families to promote comfort
