Do you enjoy working in an exciting and challenging environment? We are looking for a part time Supportive and Palliative Care Coordinator to join us in Royal Surrey Cancer Centre, Guildford. This is a new role and offers an exciting opportunity for someone to join our friendly and dynamic team.We are looking for an individual who can demonstrate excellent communication skills, good IT skills and ideally a working knowledge and experience of working within a patient focused role. NHS experience is desired but not essential.The successful applicant will work alongside another SPCT coordinator and be able to demonstrate that they can work upon their own initiative whilst working well within a team. The role will include coordinating care of patients with life limiting conditions, and offering support under the supervision of the SPCT clinical nurse specialists. The role will require communication with all members of the SPCT and wider MDTs. Being the first point of contact for the team, patient queries will be an essential part of the role and will therefore require an empathetic and can do attitude. This is a great opportunity to combine an administrative post with some patient facing activities. Main duties of the job
Under the direction, guidance and supervision of the Supportive and Palliative Care Clinical Nurse Specialists (CNSs) and Enhanced Supportive Care project manager, the post holder will help coordinate the care, and offer support to patients with life limiting conditions. They will perform administrative duties for the team in order to ensure the smooth running of the service.
Coordinate care and support people with life limiting conditions who are under the care of the supportive and palliative care team. Perform administrative duties to enhance the delivery of an efficient service.
To have made an effective contribution to reaching the Trust's vision, strategic objectives and key work programmes.
