Job summary
We are making some changes in the way we deliver our services across Primary Care, Community services and other organisations.
As we launch our Integrated Neighbourhood Team and our Transfer of Care Hub, we're building a new way of working that keeps patients safe, supported, and living well in their own homes. These teams will help prevent unnecessary hospital admissions and enable faster, smoother discharges--by wrapping expert community care around each patient.
We require an experienced administrator to join us to provide the essential administration and organisation for us to deliver our ambition. You will be working with a range of clinicians who are all focused on delivering and making a difference to patients .

Main duties of the job
The postholder will provide high-quality administrative support across the Transfer of Care Hub and Integrated Neighbourhood Team (INT) Hubs to ensure smooth co-ordination of hospital discharges and effective functioning of community-based, multidisciplinary services. Working closely with clinical leads, social care teams, voluntary sector partners, and operational managers, the postholder will support the development of integrated working practices in community services, robust discharge planning, and timely communication to improve patient outcomes and system flow.
The two postholders will work closely together where one will have a primary focus on ToCH admin support and the other INT admin support. The postholders will be required to cover for each other in times of leave.
