Job summary
Royal Surrey NHS Foundation Trust is seeking a visionary and experienced healthcare leader to oversee our Integrated Transfer of Care Hub (ToCH). This is a strategic and operational leadership role focused on improving hospital discharge pathways ensuring timely, safe transitions from acute care to community services.
As the ToCH lead, you'll lead a multi-agency team, collaborate with system-wide partners to improve patient flow, reduce delays, and enhance the discharge experience in line with the national Home First principles. This is a unique opportunity to shape integrated care delivery and improve patient outcomes across Guildford and Waverley.

Main duties of the job
The main purpose of this role is to lead the Integrated Transfer of Care Hub and the services within it, managing them as one integrated team under a single management structure. The post holder will have operational and strategic accountability for timely discharges from acute settings to community health and social care providers. The postholder is required to travel across sites and must have a full driving licence and access to their own car.
They will provide daily oversight of system flow, including capacity and demand, and escalate issues when required. Working closely with system wide Health and Social Care Senior Operational Leads, the post holder will ensure flow across discharge pathways (Pathways 0-3), expedite discharge times, minimise barriers, and improve patient experience.
The role involves monitoring, reviewing, and reporting performance related to discharge and delays, and working with system leads to develop and implement robust, collaborative systems across partner organisations.
The post holder will lead the evaluation and continuous improvement of the service to enhance discharge pathways and support regional and local goals, including reducing the number of patients with no reason to reside in hospital.
This role includes senior clinical oversight of hospital discharge pathways, supporting teams to ensure efficient, effective assessments and interventions.
