Frailty Same Day Emergency Care & Frailty Virtual Ward/Integrated Frailty Crisis Pathway

Reporting to: Integrated Frailty Assistant Service Manager

A 6 month secondment opportunity has arisen for an MDT Pathway Coordinator within Integrated Frailty, working across RSCH Guildford and Milford sites.

This position is part of the team providing administration support to our Integrated Frailty clinical teams. The main responsibilities are:

o Onboarding/discharging patients referred to the Frailty Same Day Emergency Care (FSDEC) and Frailty Virtual Ward (VW) using the Hospital Electronic Records system

o Coordinating communications between patients, their carers' and healthcare professionals across primary, hospital, social and community care services

o Tracking patients' care pathways to ensure frailty waiting time targets are met.

o Developing, supporting and coordinating Multi-Disciplinary Team (MDT) Meetings.

o Preparing information to report on activity within the FSDEC and VW

A significant level of liaison with different departments is required and the successful candidate needs to have highly developed diplomatic and influencing skills. Attention to detail, excellent communication and organisational skills are key to success in the position. Knowledge of EMIS, Millennium EPR and medical terminology, especially relating to frailty, is essential.

Hours per week: 37.5 | Monday - Friday | worked, as required, between 08:00 and 18:00

Main duties of the job

The Integrated Frailty Pathway MDT Co-ordinator will ensure that frailty multi-disciplinary meetings are co-ordinated efficiently and effectively, patient pathways are actively tracked to meet waiting time standards, support the production of evidence for national activity reporting.
