Job summary

Surrey and Borders Partnership (SABP) is looking fora Community Mental Health Nurse to join their team. You will To assess, plan, implement and evaluate the nursing care needs of a specific group of people who use our services at Surrey and Borders Partnership Foundation NHS Trust. this will also include mentoring, supervising qualified and unqualified staff while also participating in recruitment, deployment and management of staff.

General shift timings: Monday to Friday from 9AM to 5PM

Working hours: 37.5 hours Per week

Main duties of the job

  • Lead the clinical management of care packages for service users.

  • Coordinate, deliver, and monitor person-centered care, including physical and psychological health needs.

  • Deliver therapy (including CBT) both individually and in groups.

  • Work as part of a multidisciplinary team (MDT) to assess, plan, and review care.

  • Ensure the provision of physical care (nutrition, hygiene, rest, activity, physical observations).

  • Provide therapeutic activities and emotional support, while maintaining professional boundaries.

  • Carry out and respond to clinical observations (BP, pulse, weight, blood sugars, etc.).

  • Undertake risk assessments and implement risk management strategies.

  • Safeguard vulnerable adults and children, escalating concerns appropriately.

  • Maintain accurate, timely clinical records in line with policy.

  • Lead on developing, maintaining, and monitoring service standards, contributing to audits and service improvement.

  • Supervise, mentor, and support staff, students, and new starters.

  • Support recruitment, training, appraisals, and staff management processes.

  • Ensure compliance with professional standards, Trust policies, and legal frameworks (NMC code, health & safety, confidentiality, safeguarding).
