Clinical

The clinical commitment is based on the intensive care unit at Royal Surrey County Hospital and will include day-shifts, night-shifts and weekend shifts. There is excellent presence of senior cover - with consultants on site until 10pm every day and airway-trained colleagues readily at hand 24/7. The Unit is adjacent to the operating theatre suite.

There are 28 flexibly staffed intensive care beds and dedicated side-rooms, and the unit runs at a high occupancy rate. It has undergone a rapid expansion in the last five years, associated with the expansion of specialist cancer services. Over 1300 patients are admitted annually with ~60% coming from surgical specialties. The unit has paperless electronic records. High quality ultrasound is available throughout the hospital departments with state-of-the-art machines in ICU, theatres and the ED.

Non-Clinical

Depending on the interests of the candidate we can accommodate projects either in clinical research or clinical informatics.

Research

We run clinical trials in acute medicine, critical care, and perioperative medicine. There are opportunities to gain training in research conduct, governance, ethics, as well as recruiting into studies. There are the following opportunities:

- experience setting up new studies/trials in anaesthesia / critical care / perioperative medicine working with the research delivery team

- attend a Research Ethics Committee meeting as an observer to understand the processes.

- attend meetings of the local Clinical Research network (CRN)

- spend time with the R&D governance team to gain insights into governance and set-up / reporting processes.

- spend time in the local Clinical Trials Unit (Surrey CTU)

- attend national critical care research conference (UKCCRF) and ESICM LIVES (both paid for from research funds if study budget does not cover)

- attend medical statistics course (IRL or online)

- receive training in systematic reviews (IRL or online)

- co-author articles for publication in peer-reviewed journals

If you wish to stay longer than 12 months and become Research Active:

- collaborate and design a study/trial

- seek funding, get help with the IRAS process, and portfolio adoption status

- run your own study/trial

- By mutual agreement the post can be extended to post-holder to undertake an MD or PhD at the University of Surrey (minimum 2 years for MD, 3 for PhD)

- Monitoring (NIRS, continuous cardiac output)

- Immunodiagnostics in sepsis

- Functional recovery from severe community acquired pneumonia

- Machine Learning algorithm for expediting diagnosis of abnormal haemodynamics

- Acute Kidney Injury biomarkers

- Perioperative microcirculation

- Perioperative fluid resuscitation and post-operative vasopressors

- Role of the enteric microbiome in post-operative complications

- Remote ischaemic preconditioning

- Muscle wasting: perioperative and prevention of ICU-AW

- Endothelial cell biopsy in shock state, translational proof of concept study

Person Specification

Qualifications

Essential

• Full GMC registration with a licence to practice
• FRCA or equivalent such as MRCP or MCEM
• Experience and training in intensive care medicine

Desirable

• Experience and training in anaesthesia

• Previous experience in clinical research
