Would you like to join our developing Intermediate care team? We work as part of Integrated Community services in Guildford and Waverley and are looking to recruit Band 2/ 3 Health Care Assistants to support our Community patients.

You will be supported with training and career development opportunities which includes a Nationally recognised care certificate and local team competency programme. You will have the opportunity to develop your expertise in a community setting with patients with a range of conditions.

Main duties of the job

We are passionate about supporting patients at home, preventing unnecessary hospital admissions and supporting people when they are discharged from hospital. You will become part of our team working with Occupational Therapists, Physiotherapists and Nurses to provide short term care and rehabilitation to patients in their own homes to help them regain their independence. You will deliver care to patients whilst also supporting their rehabilitation programme as overseen by the registered staff.

You will be supported with training and career development opportunities which includes a Nationally recognised care certificate and local team competency programme. You will have the opportunity to develop your expertise in a community setting with patients with a range of conditions.

Hours: We operate 7 days per week 8 am - 9 pm.

Knowledge and Skills

Essential

  • Interest in working in community setting (patients' own homes)
  • Numerate and literate Standard keyboarding skills
  • Ability to work as an effective team member
  • Ability to work on own initiative
  • Effective written and verbal communication and interpersonal skills
  • Willingness to undertake training as appropriate to the post
  • Frequent requirement to exert moderate physical effort for several long periods during a shift
  • Concentration is required when carrying out personal care and clinical procedures, driving
  • Able to cope with occasional exposure to distressing or emotional circumstances; chronically sick or disabled patients
