Respiratory Clinical Nurse Specialist- Band 6:
37.5 Hours per week
12 month secondment considered for internal applicants
We are looking for a full time Respiratory Nurse Specialist role to join the Integrated Respiratory Team at the Royal Surrey Hospital.
We are looking for a dynamic, enthusiastic, highly motivated Respiratory Clinical Nurse Specialist to the join the team.
This is an ideal opportunity for those who want to develop or further enhance their specialist respiratory knowledge and skills within a supportive environment.
The successful candidate will join a multi-disciplinary team of Respiratory Nurses, Physiotherapists, Occupational Therapist and Consultants and will have the opportunity to develop and enhance their respiratory clinical practice.
The Integrated Respiratory team successfully work across both hospital and community settings, providing specialist acute inpatient care and ongoing community support, providing specialist respiratory care outside the hospital walls.
Main duties of the job
The IRS supports admission avoidance, early supported discharge, outpatient clinics, community MDT clinics, Virtual MDT clinics, community home assessments and home oxygen assessment & review. Providing specialist respiratory care to patients in the right place at the right time.
We operate Monday-Friday and cover Guildford & Waverley Place.
Car Owner/Driver is essential.

KEY RESULT AREAS/MAIN DUTIES AND RESPONSIBILITIES:
- Clinical
- Manage specialist care for patients with chronic Respiratory Disease across agreed clinical pathways.
- Inpatient assessment and review whilst on the wards, providing advice, support, and medication reviews.
- Identify patients suitable for fast track/early supported discharge and COPD discharge bundle.
- Support initiatives such as early/supported discharge, pulmonary rehabilitation, oxygen provision, smoking cessation, completion of COPD Admission and Discharge bundles and local Breatheasy support group.
- Participate in Respiratory Nurse-Led and Oxygen clinics.
- Develop effective communication channels and work within an integrated team to identify patients with Chronic Respiratory Disease who require support at home. This includes close liaison with consultants, A+E, EAU, all hospital wards, other specialist nurses, G.Ps, Community Matrons and other community colleagues.
- Ensure that patients, relatives and carers receive and understand information and education that enables them to optimise their health potential and quality of life minimising the impact of disease and promoting health.
- To be able to engage and communicate effectively in difficult discussions involving long term care and end of life.
- Participates in assessment and management of patients receiving long term oxygen therapy as required both in clinic and in the patients home.
- To be competent to work across primary and secondary supporting the management of patients in both acute and chronic phases of their condition.
- To maintain accurate documentation and records.
- Utilises research/evidence based guidelines in the delivery of specialist respiratory care.
