Job summary
An exciting opportunity has arisen within Cancer Services for a dynamic and proactive individual with excellent interpersonal and operational skills to work with colleagues from across the Cancer Alliance to co-ordinate cancer pathways and to ensure efficient and accurate data collection for shared patients.
You will be required to co-ordinate the cancer pathways for key tumour sites across the Cancer Alliance. Ensuring that all cancer patients referred via all cancer pathway routes are treated within National Cancer Waiting Times targets to ensure delivery of subsequent standards (if appropriate)
Supporting shared care patients through the cancer pathway as required by the patient when they are referred to the service from across the SSCA region.
Navigation of patients through the cancer pathway, ensuring timely booking and reporting of investigations, management and escalation of delays and issues in line with the optimal timed pathway and agreed KPI's
Main duties of the job
The Shared Care Navigator will use specialist knowledge of the patient pathway to proactively determine timelines and use high levels of communication to expedite appointments/procedures as necessary and to resolve issues which could lead to target failures
Coordinate care for shared care patients by providing a single point of access
Contribute to the effective communication process between RSFT Cancer Services, other departments within the Royal Surrey and those at tertiary centre's.
To co-ordinate the patient pathway with minimal supervision which includes:
Transfers of patient care to other organisations or teams following local guidance, the Surrey and Sussex Cancer Alliance IPT policy and as outlined in the National Cancer Waiting Times guidance.
Proactively ensuring investigations are booked and completed as required in line with agreed KPI's
To ensure accurate data is rapidly captured and recorded to meet the standards for the management of patients and to fulfil the cancer information requirements within specified timescales
