Job summary
The post holder will work autonomously as a diabetes specialist nurse within the wider Multi-disciplinary team providing diabetes care to an inpatient and outpatient population and those who care for them. Within the role there will be a special focus on providing Diabetes support in oncology service users undergoing systemic anti-cancer therapies and developing services in line with national frameworks
Main duties of the job
The post holder will provide expert clinical care and expertise within diabetes services. They will contribute to the strategic development of the diabetes service taking account of National framework, government, regional and trust initiatives. This should also involve agreements with primary care and the Surrey Heartlands ICS.
In addition, they should work in collaboration with medical and other multi-disciplinary staff to provide a comprehensive patient focused service. The post holder will provide nurse led clinics and ongoing education and support to service users based on current best practice. The post holder will advise on the management of and facilitate the timely discharge of patients admitted with diabetes either as a new diagnosis or as part of their ongoing condition. To participate in education programs for staff and service users and provide support and diabetes advice for all teams' hospital or community based.

The aim of the post is to provide specialist up to date evidence based patient centered diabetes care, on the wards in the outpatient department and to the wider community. The post provides an integrated link between the GP and consultant led specialist teams and inpatient and community nursing teams including practice nurses, community matrons and District Nurses.
Clinical
- To asses, plan and implement care of patients with newly diagnosed type 1 diabetes. To increase quality of care, reduce length of stay and prevent unnecessary admissions
- To work with other members of the multi-disciplinary team to provide holistic care to service users with diabetes
- Collaborate with secondary and primary care teams to enhance patient care, reducing any delays in treatment and promoting a timely discharge from secondary care.
- Has a good knowledge of the actions of insulins, GLP-1 and oral hypoglycemic agents, in order to provide advice to patients, and staff on the alteration of doses and the rationale.
- Demonstrates specialist skills and knowledge in clinical practice, acting as a resource in the provision of information and advice for patients carers and other Health care professionals.
- Develop ongoing training packages for the health care professionals within the ward setting.
- To provide continuing education and support to people both newly diagnosed and with existing diabetes type 1, complex type 2 and 3c and offer the appropriate structured education package.
- Provide written and where available, signposting to relevant electronic information for service users and carers on different aspects of diabetes care and management and appropriate education including psychological support
- Advise service users, carers and health care professionals how to use diabetes equipment and technology, including (blood glucose) BG meters, pens, pumps and data platforms.

- Providing innovative approaches to education through one to one and group settings.
- To keep contemporaneous, comprehensive and accurate records on all appropriate systems.

- Provide support and line management structure for Band 6 Diabetes specialist nurse and any junior nurses and HCAs working at Cedar.

- Influences, facilitates and supports positive change relating to the diabetes service. Contribute to recognizing the needs of the service, identifying risks and embracing change.
- Participate in management of staff absence, annual leave study leave and maternity leave to ensure cover for the service.
- To implement and maintain individualized performance reviews and objective setting for the staff line managed within the appraisal framework. Work with staff to identify training and development needs.
- To ensure the needs of the service are maintained by appropriate management of own caseload.
- To ensure all activity is documented on Cerner/Emis to ensure clinical and managerial records are maintained according to current guidelines.
- To ensure all staff comply with health and safety, infection control and COSHH regulations and that Trust policies and procedures are followed correctly.
