New Hospital at Home service, new opportunities for you

Senior Nurse Band 6:

We have started a brand new Hospital at Home service for our frailty patients, which will give some patients the opportunity to receive acute care and treatment where they feel most comfortable in their own home.

You will be instrumental in treating acutely unwell patients in their own homes, working alongside doctors, ACPs and a variety of other MDT members.

We are looking for nurses with Advanced Clinical Skills or willing to develop further to be part of Hospital at Home Team.

Are you an enthusiastic high skilled Nurse willing to take a new Challenge? Are you interested in developing your clinical skills further? Join the Hospital at Home Team. You will be supported and have the opportunity to develop your career further while in post as we will support you with a variety of courses.

This role will involve an element of driving and whilst prospective employees may be able to have access to a lease car we recommend you will need to have a valid UK driver's license for this role.

  • NMC registered Nursing Qualification & evidence of Continued Professional Development (CPD)

Desirable

  • Physical Assessment at Level 6 or 7 or a desire to Complete this as CPD.
  • Mentorship qualification

Knowledge and Experience

  • Some understanding in differential Diagnosis / Physical assessment in order to formulate an impression as to why a patient may have deteriorated.
  • Has an understanding of the background to and aims of current healthcare policy/national guidance/CQC/ and appreciate the implications of this on engagement.
