Job summary

The Inreach and Home Treatment Team, based in Chesterfield, is seeking a dedicated and enthusiastic Support Worker to join our welcoming and supportive team.

This role offers a rewarding opportunity to work with older adults experiencing a deterioration in functional mental health. As part of a multidisciplinary team, you will contribute to delivering intensive, home-based treatment as an alternative to inpatient care. Under supervision, you will provide competent and compassionate care tailored to individual needs.

The successful candidate will be required to travel across a wide geographical area, covering Chesterfield and North East Derbyshire, and therefore must hold a valid driving licence and have access to a vehicle. The service operates 9am to 5pm, seven days a week, including bank holidays, so flexibility is essential.

Main duties of the job

Support service users in their homes or other agreed settings.

Assist senior staff in implementing and reviewing care plans by feeding back your findings/thoughts.

Encourage participation in therapeutic activities.

Support practical tasks outlined in care plans.

Report concerns to appropriate professionals.

Maintain accurate records.

Work flexible hours, including weekends.

Travel across a defined geographical area as needed which may include cross working if required.
