Job summary

Are you passionate about making a difference in children's lives? We're looking for a compassionate individual to join our small, supportive team within the Derbyshire Children's Continence Service. You'll work alongside dedicated nurses to help children with disabilities achieve continence where possible--or provide comfort, dignity, and support when it's not.

Main duties of the job

To work as part of the Children`s Continence Team to support children and families/ carers and their educational setting with the implementation health care plans working towards achieving continence. Carrying out continence reassessments on children and young people who require containment products ensuring that the product is suitable, meeting the needs of the child/young person and promoting continence were possible.

To support the nurses in developing care plans and resources to support children with delayed toilet training and other continence related issues.

Under the direction of the nurses, support families / carers to implement plans and re-evaluate to ensure progression.

Meet with Teachers /SENCO to discuss care plan implementation within schools.

Establish and develop multi- agency working relationships

Liaise with other community and universal services as requested by the nurses

Organise and run continence reassessment clinics ensuring that children and young people are receiving the most suitable containment products and promote continence where possible.

To participate in the running of toilet training workshops for families.

To keep accurate, current records.

Participate in the writing of reports and attend / contribute within a variety of meetings as required.

Be aware of health related policies and procedures for the school.

To contribute to the collation, delivery and presentation of statistical information and data as required by Derbyshire Healthcare Foundation Trust to meet the NHS Agenda.

Comply with Trust policies and procedure.

A clinical supervision programme will be offered to the post holder.

Participate in safeguarding children, ensuring their compliant with training, policy and procedure in regards to this matter.

Ensure mandatory and role specific training is completed.

Participant within their personal appraisal process.
