Job Purpose

The Living Well programme is transforming adult and older adult community mental health services in Derbyshire. In line with the Community Mental Health Framework (CMHF), we have mobilised an integrated service across 8 localities that is delivered in partnership with health, social care, and the voluntary, community and social enterprise sector (VCSE). This is an exciting time to join the Programme Team, as we move into the next phase of the transformation programme and focus on key areas of work to develop further.

Main duties of the job

As Pathways Lead, you will lead on the development and implementation of effective and robust pathways between Living Well and wider internal and external services, under the guidance of the Living Well Practice Lead. This role will provide leadership within the programme team to identify potential pathways, develop processes for people to move seamlessly between services, and to support the implementation and embedding of new processes. The post holder will be expected to work autonomously within the Programme Team, providing clinical experience to support the development of pathways that are safe, effective, and meet the aims of the Community Mental Health Framework.

The Living Well Pathways Lead is a system role that will require close working with Living Well System Partners, this includes DHCft, Adult Social Care, and VCSE. Key duties include:

1) act as a positive role model to support the culture change set out in the Community Mental Health Framework, demonstrating strong leadership skills and advocating the benefits of the transformed service model

2) provide clinical advice to optimise the quality and safety of pathways between Living Well and wider mental health services. This may involve working alongside operational and clinical leads to understand challenges in pathways and processes, in particular where practitioners are experiencing difficulties in supporting people to move seamlessly between services

3) analyse and respond to complex technical processes, utilising specialist knowledge to assess a range of options to formulate solutions and recommendations

4) act as a change agent, using skills that will motivate and reassure operational and strategic leads through negotiation, and facilitating the change process with practical support such as assisting with clinical systems configuration

5) ensure the implementation and understanding of the Community Mental Health Framework

6) support the Living Well Strategic and Practice Leads with wider engagement activities within DHCft, across the system and the wider community. The post holder will support the promotion of the transformed service model, receive feedback from stakeholders, and horizon scan for new and emerging opportunities to provide joined up care between Living Well and wider organisations

7) to develop, monitor, and update relevant Living Well programme plans

8) identify skills and knowledge gaps across the pathways and to respond accordingly, and to make complex judgements requiring analysis, interpretation and comparison of options by integrating complex knowledge sources in new and /or unfamiliar contexts

Qualifications and Professional Registration

Essential

  • Registered Nurse, Social Worker, AHP or equivalent professional qualification
  • Degree/ Diploma in Nursing, Social Work, or equivalent professional qualification

Desirable

  • Project Management Qualification or equivalent level of experience
  • Certificate in Management

Experience

Essential
