Job summary

We have a fabulous opportunity for an RMN to join the busy and dynamic Chesterfield Central Older Adult Community Mental health Team.

We are looking for a clinician who has a specific interest and passion for the care of older adults.

The team provides assessment, treatment and care for patients who experience organic and/or functional illnesses, as well as support to their carers.

Main duties of the job

The role of a CPN can be challenging and varied with the management of a caseload, which will include patients with both functional and organic diagnosis.

You are likely to support patients by administering depots, monitoring medications, providing support from various psychological approaches, and supporting people to access services or support in the community.

The role also involves attending CST meetings within local GP surgeries, MDT meetings, supporting students and nursing assistants, as well as the wider team.

The wider team is very supportive and nurturing, and there will be opportunity to shadow nurses and OTs in their daily duties in order to expand your knowledge and skills - for example, shadowing duty or an initial assessment.

The successful candidate will need excellent communication skills, the ability to complete safety assessments, be able to produce person-centred care plans.
