Job summary
The post will be responsible for the process to access inpatient care for patients within the Acute Service for Adults of Working Age Division.
The post holder will be responsible for the efficient and effective management of the patient journey through the Trust and external services.
The post holder will report to the ASM for Hartington Unit but will be expected to work closely with the ASM for Radbourne Unit.
The post holder is responsible for receiving referrals for patients who have a registered GP in
Derbyshire and who have been gate-kept for an admission by Crisis & Home Treatment Team (CHTT).
The post-holder will be required to coordinate admissions for adult Acute patients in to DHcFT beds, or other beds (out of area/PICU) as required.
The post holder will work as part of the flow team to ensure that patient flow within DHcFT maximises the use of available inpatient capacity whilst maintaining patient safety.
The post holder will also focus on discharge delays arising out of health and socialcare needs by managing the transition/discharge nurses and team looking at accommodation, benefits, Home Office and asylum seeker issues - toinclude the identification of any overseas visitor in relation to charging for NHS care.
Main duties of the job
To manage the adult acute inpatient bed resource for DHcFT which currently sits across two sites; one in the north of the county - The Hartington Unit (consisting of 48 adult acute beds ) and one in the south of the county - The Radbourne Unit (consisting of 80 adult acute beds). The Bed Manager will support the bed management team in looking at where the most appropriate admission bed is available and work with the ward teams and early discharge staff to ensure continuous FLOW across the inpatient areas. They will work with system partners and multi-disciplinary agencies to support with access to beds when required following Mental Health Act assessments and keep the Emergency Department pathway flowing .
