Killamarsh Older Adult Community Mental Health Team (CMHT) & the North East Derbyshire and Bolsover(NEDB) Living Well Team are looking for an enthusiastic Band 6 Older Adult Specialist to join their friendly and supportive team based in Killamarsh.

You need to have experience of working with older adults with mental health conditions, in a community setting.

This newly-developed role, hosted by the Older Adults CMHT will involve supporting introductions into the NEDB Living Well Team, identifying the needs of older adults with mental health needs who live within the local community, designing & implementing person-centred support to enable people to live their best life.

This will include working with colleagues across the local community to ensure that we provide the right service at the right time by the right person.

The role is supported by the Killamarsh Older Adults CMHT, but working into the NEDB Living Well Team, covering the Killamarsh locality. There will be an additional Older Adults Practitioner in the NEDB who will cover the Bolsover and Clay Cross locality.

NEDB Living Well is a locality-based, multi-agency team, offering seamless support from agencies by placing the person at the centre of their support needs.

You will need to have the ability to travel across the geographical area

Main duties of the job

A key feature of the role is to support developing the Living Well offer for Older Adults to ensure this population can take advantage of this approach.

A core aspect of the framework is supporting those who may not have been eligible to access secondary mental health support previously.

As outlined in the Community Mental Health Framework the offer is for people experiencing a functional mental health presentation.

It is essential to have knowledge of organic conditions, ensuring people receive support from appropriate services.

A key element of the role is to identify how working into the High Peak Living Well Team as an older adult practitioner can be most effective and how that team can be an extension of the Older Adult CMHT. This includes developing links with community organisations.

The role involves working within the Living Well approach, values and practice.

You will support individuals to improve their mental health by helping develop well-being skills, build resilience and utilise community resources.

You will use skills from your professional background.
