Job summary

An exciting opportunity has arisen for an enthusiastic individual to undertake the role of a full time, Band 4, Lead Administrator for the Erewash Living Well Project.

The Post holder will provide high level, comprehensive administrative support to the Living Well Team in Erewash, to assist with the planning, design and delivery of the project.

We are looking for an enthusiastic individual, with excellent communication and interpersonal skills, as they will be liaising with a wide variety of people, services and organisations, both internal to the Trust and from numerous external organisations.

The successful post holder will need the ability to deal with complex information and to have excellent IT skills to be able to create, develop and maintain spread sheets, databases and other electronic systems to capture and maintain overall accurate project data.

Main duties of the job

1. Co-ordinate and manage the administrative functions of the Daily huddle meetings. Contribute and participate in prototyping within the Living well model.

2. Manage the database to track service user's case management, journey and outcomes, and undertake analysis of caseload information for audit and performance management purposes - to be reported back to practices/CCG.

3. Receive collate and present information from primary and statutory services including out of hours calls, and present this information to the daily huddle

4. Point of contact for the team for service users and carers and co-ordinate with other parts of the system with and on their behalf.

5. Complete welcome calls for people new to the service to determine their needs and wish's, co-ordinating their journey - providing and confirming relevant details. This can include non-clinical interventions such as making onward referrals to VSCE services.

6. Signpost team members, service users and carers to relevant service,

7. Proactive use of available software systems - contribute and lead on the System 1/1EPR projects.

8. Building and contributing to the Living well networks, fostering relationships and encouraging new ways of working with partners.

This team will co-ordinate from different sites , but you will be based at Ilkeston Resource Centre.

There may also be some working involved.

  • Educated to or working towards NVQ Level 3 or above in care or equivalent qualification/training
  • RSA III Typewriting / Word Processing (or equivalent qualification or demonstrable experience erience). oBasic computer skills. oA portfolio of relevant training/ short courses appropriate to post.
