Main duties of the job

The post holder will:

  • sustain a respectful, consistent, and reliable therapeutic relationship with service users/carers/families and work in a short-term, solution-focussed way.
  • have excellent triaging skills and offer support and guidance to all who contact us and be able to work as lead professional for our service users during their time with us as well as working together within the team.
  • be a role model for others; supervise students (with training provided as necessary) and may lead in a certain specialism/area.
  • work as duty worker for the team at times and work well under own initiative as well as within the team, being able to conduct visits in patients own homes or in care homes within High Peak and Dales.
  • undertake comprehensive initial assessments, including risk screening, positive risk taking and exploring options around least restrictive practice with the aim of the team being to reduce hospital admissions and maintain people living with Dementia or cognitive impairment in the community where possible.
  • work with service users and carers through their difficult time, empowering and enabling some control over their needs and/or explore areas to support wellbeing, to include interventions to reduce and mitigate risks.
  • support service users both in their own homes and in community settings, eg care homes and hospital settings for in-reach support.
  • explore educational techniques and sharing knowledge and skills with service users, carers, and other professionals

The service consists of a multi-disciplinary team working with older adults with a diagnosis of Dementia or a cognitive impairment.

The service acts as an alternative to hospital, facilitating early discharges from the acute setting and preventing hospital admissions wherever possible and appropriate.

The team will work with a shared caseload, providing short term, intensive interventions.

We are looking for an experienced, mental health nurse to be part of this excellent service, someone who is focused on making a positive difference to peoples lives.

As a Lead Nurse, you will be able to make complex decisions, lead a shift, cover duty, and formulate detailed assessments and person-centred care plans to assist in the prevention of hospital admissions where appropriate.

  • Personal development will be supported and encouraged through Appraisal, regular supervision, and access to training opportunities.
  • The post holder will be clinically responsible for supporting the delivery of a flexible, responsive local service within the community.
  • The post holder will travel independently across the catchment area of the service and occasionally outside of this if required.
  • The service operates 7 days per week, the post holder will work a shift pattern facilitating this which includes working from 8am-8pm, Mon-Fri and 9-5pm at weekends/Bank Holidays.
  • The post holder will undertake the Duty role, triaging referrals, allocating visits, preparing, and delivering handover, ensuring colleagues are "safe and well".
  • MDT working; liaising with a multitude of different professionals and agencies.
