Job summary

This is an exciting opportunity for the right candidate to work as a Team Lead across our three special Schools in Derbyshire. These Include Ivy House, Alfreton Park and St Giles Spencer Academy.

You will carry the operational and clinical responsibility working alongside the school head for the assessment of health needs and co-ordinate the development, implementation and evaluation of packages of care for the children who attend the school.

You will need to have excellent communication skills and be able to operate as an integrated member of the multidisciplinary team, whilst maintaining a person centred focus.

The post holder will work, in partnership with the Health and Education team to ensure the highest level of clinical practice in assessment, case management, care coordination and treatment.

The post holder will be required to ensure that the best evidence-based care is given to children, young people and their families, within the resources available to the service.

The post holder will provide operational and clinical management into to the team, supporting evidence based practice to the highest possible standard, mindful of national and local policies, guidance and legal frameworks.

Main duties of the job

The post holder will be operationally and clinically accountable for the direct delivery of the clinical care of the Special schools team. Including Ivy House, St Giles and Alfreton Park.

To act as a role model to provide high standards of professionalism representing Derbyshire Healthcare NHS Foundation Trust in the exercise of their duties.
