Job summary

We have an exciting opportunity for an experienced children's nurse to join the children's community nursing team on a fixed term basis . Our versatile and responsive team work with children and young people at home, special schools and other community settings. They provide direct nursing care, support and education and training to children and families with a variety of medical conditions and complex needs.

The team support children with acute and complex conditions and support children and their families with life limiting conditions.The team support Paediatric Virtual Ward within the community setting .

Main duties of the job

The applicant must be dynamic, dedicated, self-motivated, and able to work well independently but also within a team. Excellent interpersonal and communication skills are essential for the role as the successful candidate will work with children, young people and families and the wider multidisciplinary team in the community.

As a band 6 nurse the applicant will be expected to deputise for the team leader when necessary and supervise junior members of staff.

The applicant must be either an experienced senior band 5 or currently a band 6 nurse with Intravenous/central line competency. Experience of ward and ideally community setting .

The post holder will hold the Community Specialist Practitioner Course qualification or be willing to undertake this as the opportunity arises

The post holder will be based at Newton Drive Health Centre working alongside colleagues from various health professions. They can expect to be well supported via various routes including Trust induction, local team induction programme, clinical supervision, and Team meetings, Appraisal / Personal Development Review / Personal Development Plan etc.

The post holder will be responsible for the clinical care and treatments of children and young people within the Childrens Community Nursing Team .

The team support children and young people age 0-16 years and up until 19 years for those with SEND

The post holder will support young people in the process of transition to adulthood services and ensure they are fully supported with their required onward health and care needs

The post holder will support with the coordination of Virtual ward and participate in daily ward rounds to support early discharge from hospital and provision of responsive follow up care at home

It is expected the post holder will take at least one lead area of responsibility within the team, championing this work, reviewing National Guidelines, Standards or NICE and maintaining a quality and research driven service/care.

The post holder will support directly with nurse lead clinics and new team initiatives and undertake the required audits to demonstrate effectiveness of new practice developments

The experienced post holder will directly support junior staff and students ensuring strong effective mentorship and guidance

They will develop and deliver comprehensive teaching and training to education and nursery settings

They will directly support children and staff within our 4 local special schools ensuring children are well supported with their health and care needs comprehensively understood by the staff caring for them within their school day

The post holder will be flexible with their hours to provide support before school and in the evening, including contributing towards a 24/7 palliative care service.
