Job summary
We have an exciting opportunity for two experienced mental health practitioners to become part of the Primary Intermediate Mental Health Service in Blackpool. We are a fully integrated multi disciplinary service providing multiple treatment pathways. We work alongside our local partners to provide specialist support into various services including Early Help, Perinatal, Social Care, Family Safeguarding Hubs, Rough Sleepers teams.
Main duties of the job
The post holder will be specialist key member of the Primary Intermediate Mental Health Team, ensuring the smooth running of the service duty team and liaising/partnership working within the Blackpool neighbourhoods and locality teams. Providing effective triage and assessment, formulating and providing intervention to people who are referred with moderate and complex mental health difficulties.
Liaison with various multi agency professionals from both statutory and voluntary organisations. Sign-posting referrals as necessary to ensure that clients access the most suitable care in a timely manner.
To conduct comprehensive Mental Health and Social Care needs assessments that provide a robust treatment plan and signposting to other agencies.
The post-holder will have a sound working knowledge of the Mental Health Act 1983, The Care Act, relevant documentation and knowledge of Primary and Secondary Care Mental Health Services and Social Care.Informal enquiries welcome.

- Professional/Clinical Qualification e.g. Registered Mental Health Nurse Qualified Social Worker Occupational Therapist Affiliated Registered Profession
Desirable
- Specialist qualification or equivalent level in skills development
- Community Practitioner Degree/Diploma or similar qualification
WORK HISTORY/WORK EXPERIENCE
Essential
- 2 years' experience as qualified practitioner
- 1 year Mental Health Community experience
- Ability to and commitment to integrated working
- Previous multi-disciplinary management experience

- Evidence of recent personal/professional development
