Clinical nurse specialist in Diabetes

A fantastic opportunity has arisen to join the Diabetes Nursing team at Blackpool Fylde and Wyre Teaching Hospitals based in Blackpool Victoria Hospital. The Diabetes Specialist Nurse will be providing evidence based clinical advice to healthcare professionals, patients and their carer's in both the in-patient and out-patient setting.

The Diabetes Department are looking for an enthusiastic member of staff to join our team of experienced Diabetes Specialist Nurses to drive forward quality care for patients with diabetes. The role can be potentially wide-ranging and will be tailored to the skill and experience of the applicant.

The role carries some educational responsibility, within both one to one and group environments.

There will also be opportunities in management, service development, research and audit.

The post will involve working within a multi-disciplinary team; liaison with acute and community colleagues and will allow considerable autonomous working.

We are looking for the successful candidate to enhance and help lead our team of nurses who provide clinical nursing expertise whilst maintaining a high standard of care within the service. They must also be willing to undertake any relevant training. The successful candidate must be committed to clinical quality and continuous improvement.

Main duties of the job

To contribute to providing an effective and high quality patient-focused service that is sensitive to the needs of the users and providers with a particular emphasis on inpatient services.

To plan and co-ordinate the management and care of people with diabetes, providing education and support to empower patients or family members/carers with the skills and knowledge necessary to attain independence and self management of their condition.

To be accessible to patients, family members/carers and ensure the support for them whilst in hospital or on discharge via the telephone.

To promote evidence based practice and provide leadership, support and advice to nursing and other health professionals involved in the care of people with diabetes.

To contribute to the development of staff within the Trust to raise knowledge and awareness of diabetes and related complications.

To maintain working relationships with the consultant diabetes and endocrinology team and specialist diabetes multi-disciplinary team.

To support the diabetes consultant teams with newly diagnosed and existing patients with diabetes.

Compliment and support other roles within the diabetes team.

To participate in the development and implementation of trust standards and policies and protocols within the diabetes service.

The post holder will help to deliver patient focused services through the delivery of integrated care and contribute to the visions and values of our specialty and the Trust.
