An exciting opportunity has arisen for a full time Band 5 Community Nurse to work with our Lytham St -Annes Care home Team. This post will allow you to enhance your person centred care approach and clinical skills in an area with great career opportunities.
We require an enthusiastic individual with a willingness to embrace the opportunity to work in the community care home team.
The post holder will assist the Lytham & St Annes Care Home team with delivering and providing high quality care and support residents in line with the Enhanced Health in Care Homes model
To embed into the care homes the the principles for reducing hospital admissions and facilitating hospital discharge through collaborative working. Empower the residents and their carers through education, facilitation and promotion of self-care.
Main duties of the job
Maintain high standards of nursing care and be personally accountable for professional and ethical actions, within the N.M.C Professional Code of Conduct.

- Registered General Nurse - currently registered with the NMC
- Educated to Degree or diploma level or equivalent experience.
- Up-to-date evidence of continuing engagement in professional development and of reflective practice in a professional portfolio
Desirable
- Non medical Prescribing qualification
- Evidence of continuing professional development in chronic disease management, specifically Diabetes and COPD
EXPERIENCE
Essential
- Experience of implementing change in clinical practice
- Experience of clinical assessment and care planning.
- At least 2 years post registration experience
Desirable
- Experience of providing clinical advice within a care home setting
- Experience of working within a multidisciplinary team
- Experience of working in primary care
