Job summary
An exciting opportunity has arisen for a permanent part time post as a Health and Wellbeing Support Worker based within the supportive and enthusiastic Central Neighbourhood Team. We are a team consisting of Community Nurses, Advanced Care Practitioners, Therapists, Neighbourhood Practitioners, Health and Wellbeing Support workers and Neighbourhood Assistants.

Main duties of the job
We are looking for a caring, experienced and motivated individual to join our neighbourhood care teams to support patients using coaching techniques to better manage their own long term health conditions and wellbeing.
The successful post holder will work with others in the team. The aim of the role is to support patients within their own homes and also within care homes in order to make lifestyle and behaviour changes to improve and self-manage their health and wellbeing.

- To work as part of a neighbourhood team to support patients to improve their health and wellbeing by coaching individuals and groups to lead a healthy lifestyle as well as carrying out ad-hoc clinical interventions.
