Job summary

An exciting opportunity has arisen for a permanent part time post as a Health and Wellbeing Support Worker based within the supportive and enthusiastic Central Neighbourhood Team. We are a team consisting of Community Nurses, Advanced Care Practitioners, Therapists, Neighbourhood Practitioners, Health and Wellbeing Support workers and Neighbourhood Assistants.

Our Service Aim is to:

'Engage patients to improve their abilities, knowledge, skills and confidence working towards the better management and improvement of their health conditions and therefore their well being'

We pride ourselves on our relationships within the neighbourhood and with other statutory and voluntary agencies. You will be supported and encouraged to use all your skills to allow yourself, the team and your patients to flourish.

Main duties of the job

We are looking for a caring, experienced and motivated individual to join our neighbourhood care teams to support patients using coaching techniques to better manage their own long term health conditions and wellbeing.

The successful post holder will work with others in the team. The aim of the role is to support patients within their own homes and also within care homes in order to make lifestyle and behaviour changes to improve and self-manage their health and wellbeing.

  • Level 3 NVQ Health/ Social Care
  • Care Certificate

Desirable

  • Coaching qualification
  • ECDL

Experience and Skills

Essential

  • Evidence of relevant experience
  • Basic nursing and or therapy skills
  • IT literate and competent with e-mail, word, intranet, EMIS
  • Experience of coaching or counselling regarding life skills

Desirable

  • Evidence of successful completion of relevant recent training in e.g. equality and diversity, disability awareness, safeguarding, moving and handling, management of violence and aggression

Management

  • Ability to work unsupervised on a day to day basis and to manage/ prioritise a delegated caseload independently
