Job summary
An exciting opportunity has arisen for an experienced DSN to join the Diabetes Specialist nursing team at Blackpool Teaching Hospitals. This role is patient facing , on an inpatient and out patient basis, educating and supporting patients, families and carers around the management of their diabetes.
Main duties of the job
The successful candidate will be involved supporting patients under the 5 consultants based at Blackpool teaching hospitals. Please see main duties on job description attached.
To Provide a rapid assessment and triage service for referrals and advice requests. To promote evidence-based practice and provide leadership, support, and advice to nursing and other health professionals involved in the care of people with diabetes. To contribute to the development of staff within the Trust to raise knowledge and awareness of diabetes and related complications. To commit to provide structured education for people with diabetes if specific training is provided. To maintain working relationships with the consultant diabetes and endocrinology team and specialist diabetes multi- disciplinary team. To support the diabetes consultant teams with newly diagnosed and existing patients with diabetes. To participate in the development and implementation of trust standards and policies and protocols within the diabetes service. Contribute to the maintenance of performance targets, training and development of junior colleagues and promoting the department quality and safety policies/practice

To contribute to providing an effective and high-quality patient-focused service that is sensitive to the needs of the users and providers focusing on inpatient and community services. To plan and co- ordinate and prescribe the management and care of people with diabetes, including initiating diabetes medication, monitoring patients ongoing glycaemic control and titrate medication accordingly. To provide education and support to empower patients or family members/carers with the skills and knowledge necessary to attain independence and self- management of their condition. This will be done through the provision of face to face or remote consultations, nurse led clinics, and by supporting the specialist clinics (e.g., CSII clinic, antenatal and preconception clinic) To be accessible to patients, family members/carers and ensure the support for them whilst in hospital or on discharge through community links. To work with the wider multidisciplinary team in a proactive way with the aim of preventing hospital admission and supporting early discharge. Provide specialist / advanced skills in medicine management, assessment and disease management to hospital staff, community staff and patients through the Diabetes Help-line.
